Interested in becoming a HIV Testing Site? By becoming a HIV Testing Site, you can help increase others knowing their HIV status and help reduce new HIV transmission in Central Florida! Below you will find information regarding instructions for becoming a HIV testing Site registration and requesting a Memorandum of Agreement (MOA), and submission instructions.
Review periods will be available each quarter for interested organizations to register as a HIV Testing Site and apply to request a MOA. Documentation maybe submitted at anytime, however, all supporting documentation will be reviewed by the end of the Quarterly Review Period. Organizations should fill out the questionnaire below and submit with all supporting documentation.
Quarterly Review Periods scheduled each year:
- Available: January 1 – Deadline: January 15
- Available: April 1 – Deadline: April 15
- Available: July 1 – Deadline: July 15
- Available: October 1 – Deadline: October 15
Documentation needed to become a HIV Testing Site:
- DH Form 1781 Application Registration for HIV Testing Programs including License of certificate or number to practice in the State of Florida
- Request a CTL Memorandum of Agreement Questionnaire
- CLIA (Clinical Laborites Amendment Act) Certificate/Waiver
- Biomedical Waste Permit and Biomedical Waste Plan
- Exposure Control Plan and Needle-stick procedure (OSHA)
- Policies and Procedures relating to Confidentiality (HIPAA)
- Standard Operation Procedure (SOP) relating to Quality Assurance
- Standard Operation Procedure (SOP) relating to participation in the Rapid Start Treatment Program
- Community Outreach Plan (if applicable)
- Linkage to Care Plan for new HIV Diagnosis (CBO only)
- Linkage to PrEP and nPEP Services Plan (CBO only)
